Role summary
This role is for an enthusiastic and skilled Speech & Language Therapist to join a new Early Supported Stroke Discharge (ESSD) team in High Peak & North Dales, and also work within the adult community SLT team in Chesterfield and North East Derbyshire. You will play a key role in enabling stroke patients to recover safely at home, delivering high-quality, evidence-based rehabilitation. The position requires developed skills in communication impairments and dysphagia after stroke, as well as experience with community caseloads including dementia and neurological conditions. You will contribute to service development and clinical pathways, providing patient-focused care within a supportive multidisciplinary team.
Key responsibilities
- Join a newly established Early Supported Stroke Discharge (ESSD) team and adult community SLT team.
- Deliver high-quality, evidence-based stroke rehabilitation to enable early hospital discharge and safe home recovery.
- Work with communication impairments and dysphagia after stroke, and manage community caseloads including dementia and acquired neurological conditions.
- Demonstrate leadership skills in developing service provision and clinical pathways.
- Provide a high standard of patient-focused care and act as a strong team player.
- Travel independently across the locality and have access to a suitable vehicle for business purposes.
Essential criteria
- Recognised Speech and Language Therapy Degree Qualification
- Registered Member of Royal College of Speech & Language Therapists
- Relevant experience post registration, including dysphagia, stroke and long term conditions
- Ability to understand and apply capacity/demand issues in service area to prioritise caseload according to levels of need and explain rationale for decision making to others
- Post graduate dysphagia experience
- Demonstrates ability to be a good team member providing mentorship, clinical supervision and second opinions
Desirable criteria
- Membership of relevant Clinical Education Networks
- Experience of supervising students and assistants. Feeding back to training establishments on performance
- Experience in delivering training
