Role summary
The Discharge Coordinator team manages complex patient discharges referred to the Transfer of Care Hub. This role involves working with patients, families, and the Multi-Disciplinary Team to facilitate safe and timely discharges. Responsibilities include assessing care needs to identify suitable facilities for patients with complex discharge requirements, including those nearing the end of life. This is an opportunity for an experienced nurse to join a multi-disciplinary team.
Essential criteria
- Registered general nurse (RGN) educated to degree level (or equivalent experience)
- Current NMC
- Evidence of continuous professional development
- Evidence of having a managerial qualification or willingness to undertake courses available through the Trust to work towards one.
Desirable criteria
- Preparation for Mentorship and Preceptorship qualification (or equivalent experience)
- Knowledge and Skills
- Significant post registration nursing experience
- Previous experience of co-ordinating hospital discharges including individual care package plans in an acute hospital setting
- Experience of chairing case conferences
- Personal Attributes
- Good IT skills
- Good planning and organisational skills
- Ability to travel across locations
- Qualifications, Specific Experience & Training
