Job summary

The Dementia Intensive Support Team (DIST) based in Bury St Edmunds, are excited to welcome applications for a Senior Mental Health Practitioner (Nursing) with experience and keen interest in working with people living with dementia or complex cognitive impairment. The DIST focus on independence and implement strategies which can avoid premature moves to care homes or hospital admission.

The position offers opportunities to give hope kindness and real problem solving at a time where families and service users need it most and we are keen to welcome new faces to join our team on a permanent basis. There is potential to develop knowledge and skills, and to make a real difference to the people we serve and to the older adult offer in our Trust.

Main duties of the job

Working in a busy and dynamic Community Team means every day will be different. You will be involved in (not an exhaustive list):

  • Managing a complex caseload
  • Undertaking triage duties and initial urgent/routine assessments on behalf of the service
  • Attending multi-disciplinary team meetings with our Consultant Psychiatrist, Clinical Team Manager, Senior Practitioners, Assistant Practi
