Role summary
This is an opportunity for an enthusiastic and motivated youth worker to join the Barts Health NHS Trust High Risk Young Adult Service. The role involves supporting young people (age 16-25) living with long-term health conditions, particularly diabetes, heart disease, and kidney disease, as they transition from paediatric to adult services. The post holder will engage with young people and their families, advocate for them, and facilitate access to medical care across various hospital sites in North East London. This new role offers scope to develop the service in collaboration with senior colleagues and service users.
Key responsibilities
- Provide targeted support to young people living with a long-term condition, helping them manage their own care.
- Participate in the multi-disciplinary team, contributing specific skills to the care and understanding of young people, their families, and carers.
- Encourage and help develop a peer group network of support.
- Engage and support young people (age 16-25) and their families.
- Advocate for young people and facilitate their access to medical care.
Essential criteria
- Diploma level qualification in a relevant subject (e.g. Youth Work, community work, social sciences)
- Evidence of continued professional development
- Post qualification experience of working with young people in a range of activities and settings
- A good understanding of the purpose of Youth Work within a hospital and community setting
- An understanding of the issues faced by young people with a long term health condition as they transition to adult services
- A thorough knowledge and understanding of the physical, social and emotional development needs of young people
- Knowledge experience and application of current legislation and guidance governing youth work services
- Knowledge and understanding of issues relating to child protection and cultural diversity awareness
- A clear understanding of the psychosocial impact that coping with a long term health condition has on daily living.
Desirable criteria
- Youth Work-related Degree
- Experience of working with young people in a health and wellbeing context (e.g. teenage pregnancy, drugs advice, sexual health)
- Experience of working within a multi-disciplinary team
- Experience of being involved in audit and evaluation
- Some knowledge and/or experience of counselling methodology
- Some knowledge of complex health needs

Job summary
An opportunity has arisen for an enthusiastic and highly motivated youth worker to join the innovative Barts Health NHS Trust High Risk Young Adult Service. This mobile service will comprise a Clinical psychologist, Social workers Youth Workers supporting clinical teams looking after young people living with a long-term health condition.
These exciting new posts have been developed to improve care and outcomes for young people (age 16-25) living with a long-term health condition across North East London. The post holder will support young adults and their families who may be struggling with a range of difficulties related to living with diabetes, as they make the transition from paediatric care to adult services. The work will mainly be with young adults living with diabetes, heart disease and kidney disease.
As these are new posts, the post holders will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users, and to take a leading role in the future of young people services across North East London.
The post will involve cross site working across Barts health and North East London communities: Mile End Hospital, Whipps Cross Hospital, the Royal London Hospital, and St Bartholomews Hospital and Homerton.
