Role summary
This role is for an enthusiastic and highly motivated Senior Mental Health Nurse to join the Integrated Community Mental Health Team in East Cornwall. The team provides assessment and treatment for people with severe and enduring mental health problems, focusing on recovery and promoting independence. The successful candidate will manage a defined caseload, develop care programs, and facilitate the interface between primary and secondary care services. Key responsibilities
- Be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
- Carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care co-ordination system.
- Develop the interface between primary and secondary care services so the patients/carers are offered focused and co-ordinated packages of care that are appropriate to their needs.
- Work flexibly as part of a team, to deliver innovative health and social care.
Essential criteria
- Current professional registration - NMC
- Evidence of post basic training
- Professional qualification - RMN
- Extensive experience of working with mental health patients
- Demonstrate knowledge and understanding of mental health profession with evidence of continued professional development, as defined by organisational body
- Sound working knowledge of the Mental Health Act of 1983 and other mental health legislation
