Role summary
This role involves supporting the Community Response Team at Babington Hospital in Belper, assisting with patient discharges from acute hospitals and providing short-term and interim care. You will also help patients improve independence through re-enablement. The position requires previous caring experience, a full UK driving licence, and access to a vehicle for travel between visits. Shifts operate on a 7-day rota, including weekends and bank holidays.
Essential criteria
- NVQ Level 4 or able to demonstrate equivalent knowledge gained through experience and training
- GCSE English Language and Mathematics (or equivalent)
- Good knowledge and understanding of rehabilitation /delivery of care
- Recent and relevant experience of working within adult/older people's rehabilitation setting
- Experience working as Band 3 Support Worker/Healthcare Assistant
- Community experience
- Skills & attributes
- Excellent communication skills
- Excellent manual handling skills
- Education, qualifications & training
