Role summary
This role is for a dedicated and motivated Health Visitor to join the South Derbyshire and Derbyshire Dales 0-19 Team. The successful candidate will deliver Health Visiting services within the Healthy Child Programme, manage a defined caseload, and take responsibility for safeguarding children. The position requires a SCPHN qualification in Health Visiting and V100/V150, with a focus on public health work and community engagement. The base is in Repton, requiring travel across the locality.
Key responsibilities
- To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
- To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
- To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
- To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
- To manage a team of Health Visiting support staff.

Job summary
We are seeking to appoint a dedicated and motivated Health Visitor to work within the South Derbyshire and Derbyshire Dales locality.
The South Derbyshire and Derbyshire Dales 0-19 Team have been awarded Gold accreditation for their work with Families and Communities.

The base for this post is in Repton, this location and requires travelling across the locality, therefore own transport is essential. Training days and meetings are sometimes in other parts of Derbyshire.
