Role summary
This is an exciting opportunity for a dynamic Physiotherapist to join the Bedded Intermediate Care Units at Bolton NHS Foundation Trust. The role involves working in intermediate care beds and community beds (Discharge to Assess Beds) to deliver high-quality patient-focused care. You will be part of a multi-disciplinary team, focusing on preventing hospital admissions, facilitating discharge, and promoting independence for older people. The position requires recent post-registration experience in elderly rehabilitation and the ability to travel across the service footprint.
Key responsibilities
- Work as part of the Intermediate Care therapy team, rotating across bed base units and Discharge to Assess beds.
- Deliver safe and high-quality service in residential units in the community.
- Contribute to the prevention of unnecessary hospital admission and effective discharge.
- Promote independence, especially for older people.
- Collaborate within a health and social care multidisciplinary team.
Essential criteria
- oDegree or equivalent in Physiotherapy
- oCurrent professional registration as a Physiotherapist
- oTreatment of a broad range of conditions relating to clinical speciality
- oEvidence of leading and implementing clinical change
- oSupervision and/or mentoring of staff/students.
- oMulti-disciplinary/multi-agency collaborative working.
- oAbility to communicate highly complex pathologies and diagnostic reasoning to all levels in the medical and non-medical field at the appropriate level.
- oAble to undertake assessment in difficult complex situations
- oSound knowledge of psycho-social problems and ability to gain patient confidence and co-operation.
- oAble to work independently as lead specialist, demonstrating good leadership skills.
- oUnderstanding of clinical governance framework and implications for practice including audit and research governance.
- oUnderstanding of current NHS/government policy/Public Health/NICE guidelines on organisational change and the ability to adapt these appropriately to local service needs through publication of documents.
- oChange management skills
- oAbility to work flexibly and travel across the service footprint to meet business needs
- oAble to work under pressure and constantly respond and adapt to frequent changes in circumstances.

Bolton Hospital NHS Foundation Trust and Bolton Council work in partnership to provide patient focused care in residential units in the community. The successful candidate will be expected to rotate across the bed base units and Discharge to assess beds provision to ensure the needs of the service are met.
