Role summary
This is an exciting opportunity for an enthusiastic and motivated nurse to join the Locala Diabetes Service. Main duties of the job
An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the Locala Diabetes Service.
We are seeking the skills of a Diabetes Specialist nurse who has the ability to make a difference to the health and wellbeing of individuals referred to the service.
The role requires a motivated individual who is interested in developing themselves and others to deliver high quality, effective and innovative diabetes care in the community.
You will need to demonstrate knowledge of national and local issues and be committed to partnership working with other providers.
As part of a team and working in conjunction with GP's, mainstream nursing, social and secondary care services, you will provide a patient focused, co-ordinated case management approach across primary care.
