Role summary
This is a Bank contract for an experienced Band 6 Community Mental Health Nurse (CPN) or Social Worker (SW) based in Hemel Hempstead. The role involves clinical and operational day-to-day service delivery, ensuring individualised comprehensive assessment and care management. You will be responsible for supervising junior staff, delivering quality service, and formulating/reviewing care plans and risk assessments. A valid professional registration (NMC) is required, and you must be a car driver.
Key responsibilities
- Support the Team Leader in establishing and maintaining effective inter team and inter agency communication.
- Provide effective liaison/communication throughout the care pathway to service users, their carers and others.
- Maintain effective joint working with others, developing positive working relationships and clear communications with Multi-disciplinary Teams and other professionals.
- Act as a Care Coordinator ensuring high quality care delivery at all times.
- Take a leading role in designated projects/service development as allocated.
- Undertake Approved Mental Health Professional (AMHP) duties or be willing to undertake AMHP training.
Essential criteria
- RMN or Dip/BSc (Hons) / MSc in Mental Health Nursing
- ENB 998/Preceptorship and Mentorship
- At least 2 years of post-registration experience with at least one year at Band 5
- Experience of working in acute mental health inpatient/day care services
- Experience and knowledge of working in a multi-ethnic community
- Car Driver

- Experience of working with acute mental health problems in a community setting
- Substantial IT skills

Job summary
We are currently seeking an experienced Community Mental Health Nurse, at Band 6 to work via a Bank Contract, based in Hemel. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.
The post-holder will be responsible for the clinical and operational day to day service delivery, ensuring that an individualised comprehensive assessment, and Care Management service, is effectively delivered.
Responsibilities include the clinical and operational supervision of junior nursing staff and students as delegated by the team manager and will be supporting the Team Leader.
You will be accountable for delivering a quality service that is effective and efficient in line with the organisation's objectives and performance indicators, and to contribute to strategic service development working collaboratively with other agencies.
The post holder will formulate a care plan and risk assessment, review its effectiveness and re-plan using evidence-based interventions to best effect, in partnership with the service user. You will initiate, plan and deliver effective risk management systems within the clinical area.
