Role summary
Join the Older Adults Community Mental Health Team in South Bucks as a Mental Health Nurse, Occupational Therapist, or Social Worker. Key responsibilities
- Support the coordination of the duty team, triaging referrals, planning assessments and liaison with the bed flow team, and local Psych wards.
- Respond to urgent and emergency referrals, providing crisis support for people with dementia, and brief interventions to older adult patients with mental health needs.
- Work as an autonomous practitioner as an active participant of the multi-disciplinary team undertaking specialist assessments.
- Work in partnership with service users and their families/carers to ensure the development of a treatment plan.
- Support and supervise junior members of the team.
Essential criteria
- Registered Professional Qualification (Mental Health Nurse, Social Worker, Occupational Therapist)
- Evidence of Continuing Professional Development (CPD) portfolio
- To have experience of working within a multidisciplinary team
- Formal /informal teaching including supervising student placements
- To have relevant experience in community care and have specific clinical expertise in mental health
- Experience of working in older adult mental health
- Experience of working in the speciality of Dementia
- To be IT literate
- To have excellent English verbal and written communication skills
- To be a car driver
Desirable criteria
- Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)
- Experience of supervising and managing staff
- Experience of providing leadership

We are looking for a Mental Health Nurse, Occupational Therapist or Social Worker to join our Older Adults Community Mental Health Team (CMHT) based in the South of Bucks . We are expanding the permanent posts within our duty team and are recruiting into a full time post into this dedicated team, working alongside our Duty team and Clinical Lead Nurses.
