Role summary
This is an exciting opportunity for an experienced physiotherapist to join the community-based Neighbourhood Teams in Herefordshire on a fixed-term, part-time basis. The role involves providing direct high-quality therapeutic care and support to patients in the community, including home visits, assessments, and treatment plans. You will manage your own patient caseload, demonstrating clinical autonomy and specialist clinical reasoning. The post holder will also contribute to service development and supervise other staff.
Essential criteria
- Professional Qualification to degree level or equivalent in Physiotherapy/Occupational Therapy
- HCPC registered
- Post Graduate specialist qualification/significant post graduate experience
- Skills, Knowledge & Abilities
- Broad and specialist knowledge of relevant patient conditions and therapy interventions
- Demonstration of knowledge of core and specialist Therapy skills
- Evidence of ongoing CPD in relevant area
- Experience of multi-disciplinary communication, assessments and record keeping
- NHS or equivalent experience e.g. in inpatient areas, acute or community or community services
- Experience of using/fitting adaptive equipment
- Experience working as Band 5 in the NHS
- UK Driving license and access to vehicle

Job summary
Wye Valley NHS Trust has an exciting opportunity for**** an experienced physiotherapists to join our community based Neighbourhood Teams across Herefordshire on a permanent basis. The post holder will work out of the Leominster Community Office base.
You will be working alongside other Physiotherapists, Occupational Therapists - registered and non-registered, District Nurses, ACPs and alongside Home First reablement service and care agencies, completing home visits and offering physiotherapy assessments and treatment plans to patients. The role includes community rehabilitation, discharge to assess and urgent response.
Our community teams, we strive to invest in your development and offer regular training, team meetings, shared learning and regular supervision sessions. We aim to support the Trusts focus of providing care closer to home.
