Role summary
This is an exciting opportunity for a Senior Community Nurse to join the Peterborough City Team. You will work autonomously within an integrated multi-professional team, managing your own caseload and assessing complex patients. The role involves providing high standards of nursing intervention in patients' homes, supporting and educating junior colleagues, and ensuring patient and carer participation in decision-making. You will collaborate closely with GPs and other multidisciplinary team members to deliver excellent nursing care and reduce acute hospital admissions.
Essential criteria
- RGN level 1 educated to degree level.
- Post-registration qualification relevant to district nursing (DNSPQ or equivalent: Community Nursing Degree/ comprehensive community nursing CPD)
- Evidence of ongoing professional development
- Practice Assessor/ Supervisor
Desirable criteria
- Management of leg ulcer qualification
- Prescriber or willing to undertake
- Wound care qualification
- Continence qualification
- Multidisciplinary working experience
- Management experience
- Relevant and proven experience post registration in a variety of settings
- Nursing in both acute and community settings
- Demonstrate application of clinical governance and experience of clinical supervision
- Experience in delivering training
- Skills & Abilities
- The ability to respond to changing situations
- Ability to develop effective working relationships
- Ability to exercise initiative
- Ability to prioritise and manage workload to meet the needs of the service
- Ability to innovate
- Extensive understanding of clinical supervision
- Knowledge & Understanding

- Ability to travel independently around Cambridgeshire and Peterborough area meeting required timescales. A Full driving licence and access to a vehicle is essential.

Job summary
PART TIME 30 HOURS PER WEEK
An exciting opportunity has arisen for a Senior Community Nurse to work within our Peterborough City Team.
You will be working as part of a team which includes Community Nurses (RN), Community Nursing Clinical Leads & Healthcare Assistants. You will link closely with colleagues outside of CPFT including General practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.
The successful candidate will need to have the relevant community CPD.

Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle.
