Job summary
The Eastleigh Southern Parishes CMHT team based at Moorgreen, West End Totton, near Southampton has an exciting opportunity for a full-time Band 6 Mental Health Practitioner.
If you're innovative, proactive, and passionate about providing high-quality holistic care, then we would love to hear from you. You will have a broad depth of mental health experience working with individuals who have severe and enduring mental illness.
You will be responsible for care-coordinating a caseload of service users with complex long-term mental health problems who find it difficult to engage with services. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will work in a strengths-based and recovery-focused way to support service users to meet their mental health needs.
