Role summary
This role is for a dedicated and enthusiastic Community Psychiatric Nurse within Ealing Cognitive Impairment and Dementia Services. The post holder will provide specialist assessments, reviews, and nursing interventions to service users with memory-related health needs. Key responsibilities include managing clinical caseloads, conducting risk assessments, and working collaboratively within a multi-disciplinary team to deliver high-quality care for dementia patients. - Professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.

- Experience of working with patients and their carers with complex health and social care difficulties in the designated care group.
- Experience of supervising staff.
- Experience of managing clinical caseload will include carrying out initial assessments, treatment efficacy reviews and risk mitigation assessments leading to crisis prevention
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health difficulties.
- Experience of working consultatively with professionals.
- Experience of conducting a range of clinical assessments including risk assessments.
- Experience of partnership working within and across various statutory and non-statutory teams and agencies
- Previous experience of working with carers and independent / voluntary sectors.
- Experience in working with people with dementia on the ward and in the community.
- A high level of working knowledge of cognitive impairment and dementia.
- Ability to work within a Multi Disciplinary Community Team.
- Deputise for the duty manager as appropriate.
- Have an excellent understanding of assessment and management of risk.
- Effective planning & organisational skills with an ability to prioritise own workload.
