Role summary
Join the Community Ageing Well Service (CAWS) as a Specialist Occupational Therapist to support older people with frailty and complex health needs. You will assess needs, provide person-centred interventions, and help patients maintain independence at home. This role involves working within a multidisciplinary team to promote wellbeing, reduce falls, and prevent hospital admissions. It's a fixed-term maternity cover opportunity to develop specialist practice in a service making a positive difference.
Key responsibilities
- Complete comprehensive occupational therapy assessments for people with complex physical, cognitive and functional needs.
- Develop and deliver personalised treatment and rehabilitation plans based on individual goals and strengths.
- Assess and recommend appropriate equipment, adaptations and moving and handling solutions.
- Support people to maximise their independence, function, confidence and quality of life.
- Work collaboratively within a multidisciplinary team to develop coordinated, person-centred care plans.
- Provide specialist occupational therapy advice to patients, carers and other health and social care professionals.
- Contribute to falls prevention, frailty management and admission avoidance initiatives.
- Provide supervision, guidance and support to therapy assistants and junior colleagues, where appropriate.
- Contribute to service development, quality improvement and clinical governance.
- Build effective working relationships across health, social care and voluntary sector services to support joined-up care.
Essential criteria
- Diploma/degree in Occupational Therapy
- Current HCPC registration
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Falls, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation, Palliation
- Substantial post registration experience as a Band 5 Occupational Therapist
- Experience working at a Junior level in a wide range of specialised clinical areas, providing care to frail and elderly patients with long-term conditions and/or complex health needs including outpatients, neurology, rehabilitation, orthopedics and respiratory
- Experience of negotiating and liaising within a multi-disciplinary team in NHS and social services
- Skills in the assessment and treatment d, identifying any risk or potential problem and act appropriately effective record keeping, with adults with complex conditions and multi-pathologies
- Knowledge and understanding of complex medical conditions including multipathologies and falls.
- Ability to apply core Occupational Therapy skills across a range of clinical conditions within a client centred, goal setting.
- Knowledge of outcome measures and evidence based practice associated with Occupational Therapy
- Good communication and report writing skills, in both verbal and written format to accurately report and feedback information to colleagues and others
- Knowledge and understanding of the role of Social Services and voluntary agencies
- Knowledge of research and audit processes
- Able to work as a team member
- Able to work under pressure
- Able to resolve situations of conflict

As a Specialist Occupational Therapist, you will work within a supportive multidisciplinary team to help people remain independent, safe and confident in their own homes.
