Role summary
This role is for a motivated Clinical Support Worker Higher Level to join the team at Keswick Community Hospital. You will provide high-quality care to patients with complex needs in a community setting, working to prevent hospital admissions and promote independence. The position involves developing clinical skills, working closely with health and social care colleagues, and undertaking delegated nursing and care duties. You will often work unsupervised, assessing patients and implementing care plans.
Key responsibilities
- Work as part of a multi-disciplinary team to deliver integrated care and seamless services.
- Undertake a range of delegated patient nursing and care duties under the direction of a registered healthcare professional.
- Perform clinical observations, record patient information, and assist with personal hygiene and daily living activities.
- Assess patients, plan and implement care in the community, and provide advice and education to patients, carers, and colleagues.
- Carry out nursing procedures and maintain associated records.
- Undertake delegated management tasks, including line management of other team members.
Essential criteria
- Level 3 vocational qualification or Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3; -Theoretical knowledge -Experience of a range of care and related procedures -clinical observations knowledge of data protection legislation and health and safety at work legislation
- National Care Certificate or commitment to complete within 12 weeks of commencing employment
- Numeracy and Literacy Level 2
- Experience of working in health or social care
- Experience of undertaking and recording clinical observations and escalating changes or findings
- Working knowledge of legislation underpinning care provision
- Understanding of the importance of robust, accurate, clear, timely documentation
- Insight into behaviours and skills of a carer and the need for effective team work
- Knowledge of Safeguarding
- Understanding work-related boundaries
- Awareness of GDPR and the need for confidentiality
- Excellent interpersonal, communication and active listening skills
- Ability to adopt appropriate styles and methods of communication
- Time-keeping, planning and prioritisation skills
- Basic IT skills to use electronic patient records and other Trust systems
- Analytical and judgement skills to make decisions based on patient / service user interactions and observations and metrics
- Ability to demonstrate the importance of holistic person centred care and related procedures

An exciting opportunity has arisen to recruit a motivated and enthusiastic Clinical Support Worker Higher Level - Community - Band 3to work within Keswick Community Hospital.
ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing. This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future. These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of the local people.
This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting.
