Role summary
This is a part-time Occupational Therapist role within the urgent care response and intermediate care service in Torbay. The post holder will conduct holistic needs-led assessments, manage a complex caseload, and act as an autonomous practitioner in various community settings. The role involves working flexibly, including weekends and bank holidays, to provide community therapy and intermediate care. It also requires collaboration with various health and social care sectors to support the Care Trust's integration agenda.
Key responsibilities
- Manage an intensive acute caseload, triaging and prioritising work based on urgency and risk.
- Assess and treat patients in community settings, primarily their own homes, but also nursing and residential homes.
- Plan, implement, and review OT interventions, making autonomous decisions regarding care packages, equipment, and ongoing treatment to prevent hospital admissions.
- Conduct home assessment visits for patients from temporary places of safety to facilitate discharge planning.
- Provide supervisory responsibility for junior staff such as a Band 5 post, Assistant Practitioner, or support worker.
Essential criteria
- Diploma/Degree in Occupational Therapy
- Valid state registration with Health Professions Council
- CPD portfolio and evidence of recent post-graduate learning relevant to area of clinical practice
- Experience within the NHS including experience working in a variety of roles, for example orthopaedics, elderly care and musculoskeletal conditions
- Experience in the NHS working in a community setting
- Proven post-graduate experience
- Wheelchair Accreditation
- Pressure care knowledge and prescription of suitable equipment
- Knowledge and understanding of complex legislation surrounding health and social care
- Understanding of National Service Frameworks for Older People, specifically aspects referring to Stroke, Falls and Intermediate Care.
- Student Placement Educators Course
- Evidence of ability to make decisions under pressure and in stressful situations
- Evidence of excellent interpersonal and communication skills
- Proven ability to work on own initiative and without direct supervision and make autonomous decisions
- Evidence of ability to work within a multi-disciplinary team
- Knowledge of specialised disability equipment
- Therapeutic and manual handling skills
- Detailed understanding of roles of multidisciplinary team members/health and social care agencies
- Understanding of clinical governance issues
- IT skills
- Knowledge of vulnerable adults protocols and procedures
- Moving and handling protocols and procedures to include provision of hoists and specialist slings
- Risk assessment and management of falls
- Chronic disease conditions
- Orthopaedics
- Neurological conditions
- Elderly care / rehabilitation to include multi-pathology and dementia
- Experience of supervision of junior staff and/or OT students
- Highly developed communication skills to include negotiation and experience of communicating sensitive information e.g. unfavourable assessment outcomes

- Due to the nature of the role it is essential that applicants are able to travel independently across the locality.
