Role summary
The Neighbourhood Care Coordinator acts as a trusted point of contact for residents with health, wellbeing, and social care needs. This role involves navigating individuals through services, overcoming barriers, and ensuring access to appropriate support. The coordinator will work across various organisations, taking a whole-person approach to care, and building strong relationships with residents, families, and professionals. The position aims to improve resident experience by coordinating care, gathering feedback, and contributing to service development.
Essential criteria
- GCSE or equivalent in English and Maths, grades A-C
- Experience of working in a Health or Care setting
- Proficient in the use of health or care digital systems
- Experience of coordinating patient care
- Experience of handling difficult conversations
- Accurate data entry, typing and checking skills
- Experience of using excel spreadsheets for data management
- SKILLS, ABILITIES & KNOWLEDGE
- Proficient in all aspects of Microsoft Office
- Proven ability to work to deadlines, prioritise and multitask
Desirable criteria
- Local knowledge of community healthcare and social care
- SPECIAL CIRCUMSTANCES
- Ability to travel between sites in a timely manner if required

Job summary
We are excited to be recruiting both full and part time Neighbourhood Care Coordinators.The Neighbourhood Care Coordinators will act as a trusted, named point of contact for residents with health, wellbeing and social care needs. Working across organisations within the neighbourhood partnerships, the post holder will support individuals to navigate services, overcome barriers, and access the right support at the right time.
This innovative role focuses on taking a whole-person approach to care, recognising that a resident's health, social circumstances and personal goals are often interconnected. The Care Coordinators will work closely with health, social care and voluntary sector partners to ensure coordinated and person-centred support for people living in Buckinghamshire.
The post holders will build strong relationships with residents, families, and professionals across the system. In addition, they will play an important role in improving the experience of residents by navigating them through their health and wellbeing journey, gathering feedback, supporting people to take an active role in their health and wellbeing, and contributing to service development across the neighbourhood partnership.
