Role summary
This role is for a highly experienced and proficient registered practitioner in acute care, focusing on the management of acutely unwell patients. The post holder will use clinical expertise to deliver high-quality, evidence-based care to critically ill patients, reviewing and assessing those considered 'at risk'. They will act as a role model, providing support and training to the multi-professional team, and will have autonomy over a patient caseload across various specialties, including lone working. The role involves implementing clinical governance, modernising patient services, and participating in audit and research.
Key responsibilities
- Meet National Outreach Forum competencies and work within service specification for critical care services.
- Demonstrate the use of evidence-based specialist skills and practice in the care of critically ill patients, including effective communication.
- Demonstrate specialist knowledge and skills in the assessment, planning, implementation, and evaluation of care for critically ill patients.
- Interpret and deliver appropriate action/treatment/therapy to critically ill patients, including intra-hospital transfers.
- Monitor and evaluate patient conditions using the National Early Warning Scoring system, alerting staff to changes.
- Negotiate with the ward multi-professional team to improve clinical care for 'at risk' patients.
Essential criteria
- Current first level NMC registration or equivalent

Job summary
The post holder will be a clinically credible registered practitioner who is highly experienced and proficient in the management of the acutely unwell patient with a comprehensive background of experience in acute care. They will utilize their clinical expertise and specialist knowledge to deliver high quality evidence / research based care to the critically ill patient. They will review referred and ongoing patients and assess these patients who are considered "at risk" by the referring multi-professional team. They will act as a role model providing support, education and training to the multi-professional team caring for the critically ill patient. The post holder will have a degree of autonomy where they will be responsible for a caseload of patients across a number of specialties, to include an element of lone working.
The post holder will be expected to use their experience to support the implementation of clinical and non-clinical governance and to modernise patient services. In addition the post holder will be responsible for participating in clinical audit and nursing research.
