Role summary
This role is for an enthusiastic and motivated registered mental health nurse to join the Older Adult Home Treatment Team. The team provides intensive community-based care to older adults experiencing mental health crises, aiming to prevent hospital admissions and support discharges. The nurse will assess, coordinate treatment, and support clients and carers, working within a multidisciplinary team.
Key responsibilities
- Act as a Care Co-ordinator/Lead Professional within the Care Programme Approach.
- Manage a varied caseload, undertaking ongoing assessment, planning, implementation, and evaluation of nursing care.
- Participate in a duty system on a rota basis and contribute to multi-disciplinary service development.
- Undertake initial nursing assessments, care planning, and risk assessments.
- Deliver a range of therapeutic interventions and manage complex mental and physical health crises.
Essential criteria
- Experience of working with Older Adults in a mental health setting
- Experience of assessment and care planning
- Experience of multi disciplinary working
- Registered Mental Health Nurse qualification.
- Evidence of current or pending post qualification NMC registration.
- Good risk assessment and risk management skills including the use of assessment tools.
- Knowledge and understanding of the mental health needs of older people.
- Goal planning and service user engagement skills
- Demonstration of commitment to on-going personal & professional development
- Ability to respond to the individual rights / needs / opinions of service users
Desirable criteria
- Relevant post qualifying experience in mental health nursing
- Experience of working with older adults in a community setting
- Knowledge and understanding of the physical health needs of older adults

Location: SHSC 7 Edmund Road, Sheffield, S2
We are looking for an enthusiastic and motivated registered mental health nurse to join our Older Adult Home Treatment Team.
The Older Adult Home Treatment Team provides a service 8-8pm, 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person's own home.
You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support clients and carers. You should possess effective communication skills and have the ability to work under your own initiative and as part of a wider Multidisciplinary team.
