across Norfolk and Suffolk.

Healthcare (CHC) is a package of care, arranged and funded by the NHS, for patients

who have been assessed as having a primary health need. The CHC process

involves assessing individuals with ongoing health and care needs to determine

whether they are eligible for NHS-funded support, either in their own home or

in a residential or nursing home setting.
