Role summary
This is an exciting opportunity to join the Virtual Ward (Hospital at Home team) as a Nurse Practitioner. The role involves care coordination, discharge planning, providing clinical leadership, and supporting service development. The post holder will support patients and families with digital and remote monitoring and aid in the development of junior staff. You will work autonomously, providing expert clinical management and care to patients within the speciality, supporting the referral process from both in and out of hospital.
Key responsibilities
- Identify Patients suitable for the Hospital at home pathways ensuring timely access to the hospital at home team, either discharged from Acute hospitals or step up from community
- Act as a single point of contact for patients and health professionals to facilitate rapid access to Hospital at home services
- Work closely with other healthcare professionals in a collaborative, team-based approach to discharge to assess.
- Contribute to improving patient outcomes and service delivery within the community services.
Essential criteria
- Post registration experience, some of which will be at a senior level within the required specialist area of practice
- Portfolio of evidence of continuous professional development
- Experience of practice development, clinical education or leading change
- Experience of involvement in clinical audit
- Experience of working in Community/ Acute Setting with experience of managing frailty and or patients with long term conditions.
- Previous experience of multiagency collaboration working across professional boundaries
- Experience of practice development, clinical education or leading change in practice to upskill and support staff
- Clinically credible, with experience at senior practitioner level
- Ability to assist patients to move as per Trust Moving and Handling Policy
- Car Driver (full license, manual)
- Degree in a health-related subject and current professional registration (NMC)
- Mentorship/ Practice Assessor/ Educator qualification or equivalent
- Clinical skills accredited qualification with consolidation of skills
- Independent NMP prescribing qualification
- Demonstrable leadership skills and an understanding of own leadership style
- Excellent interpersonal and communication skills
- Evidence of ongoing professional development
- Expert assessment, analytical skills, interpretation and clinical decision making
- Ability to work autonomously within role in a complex/ changing environment
- Significant knowledge of community service provision, including social care and charity delivered services
- Knowledge and understanding of relevant national polices and guidelines relevant to admission avoidance and early supported discharge services
- In depth knowledge of frailty and long-term conditions
- Ability to effectively challenge stakeholders while maintain professional relationships
- Excellent written and verbal communication skills
- Effective time management skills

Virtual wards (Hospital at Home) work in partnership with our Recovery at Home teams are defined as a safe alternative to NHS bedded care, we are looking to expand our virtual ward team in line with the introduction of new clinical and community step up pathways in addition to the established hospital step down pathways. Hospital at home delivers care closer to home for patients, offering personalised care with technology enabled remote monitoring, providing medical and nursing, therapy and pharmacy optimisation and onward discharge referrals.
**The post holder will work collaboratively with the multi-disciplinary team.
