Role summary
This role is for a flexible and innovative Community Practitioner to join an experienced community neuro rehab multi-disciplinary team. The post holder will provide high-quality assessment and intervention skills to patients with complex clinical needs due to Traumatic Brain Injury (TBI), helping them maximize their rehabilitation potential. The role involves working in various settings, including patients' homes, the community, and residential/nursing care settings, across Northumberland. The successful candidate will develop co-produced rehab plans, deliver evidence-based clinical interventions, and provide compassionate, client-centred care.
Key responsibilities
- Develop co-produced rehab plans with patients with TBI and refer to specialists.
- Provide specialist clinical advice to Service Users/Carers, staff, and agencies.
- Regularly review and amend rehab plans with patients and families, considering Carer needs.
- Deliver evidenced based clinical interventions as a member of the Multi-disciplinary Team.
- Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.
- Provide clinical supervision to junior staff as delegated.
Essential criteria
- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
- Mentor Training.
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
- Up-to-date knowledge/experience of relevant legislation.
Desirable criteria
- Post Registration Qualifications in Psychological Interventions
- Experience of working within a community team.
- Non-Medical Prescribing
- Knowledge and Experience
- 12 to 24 month band 5 qualified nurse experience dependent upon service need.
- Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
- Understanding of difficult to manage/challenging behaviours.
- Skills and Competencies
- Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
- Risk Assessment and Risk Management / Safety Planning skills.
- Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
- Demonstrate effective written and verbal communication skills. Evidence of a high level of professional, organisational and leadership skills (self & others).
- Ability to adapt within a changing environment. Ability to work under pressure
- Ability to work effectively with other professionals using a multi-agency approach and Excellent interpersonal skills.
- Ability to work autonomously and within a team and Good conflict resolution skills.
- Clinical Leadership.
- Training & Induction of staff.

We are currently looking for a flexible and innovative Community Practitioner to join our experienced community neuro rehab multi-disciplinary team.
