Role summary
This role is for a dynamic and hard-working Clinical Nurse Specialist (CNS) to join the expanding Respiratory Department's COPD Early Discharge Team. The successful candidate will identify and assess inpatients with COPD exacerbations, supporting their early discharge and follow-up at home. The team also works to prevent avoidable admissions and supports patients requiring Non-Invasive Ventilation (NIV) in the community. Previous experience with COPD and NIV is essential.
Key responsibilities
- Work with colleagues to deliver a unified respiratory service across Swansea Bay UHB.
- Visit wards daily to identify and support patients suitable for early discharge, then follow up at home.
- Manage a caseload of patients, providing education, teaching self-management skills, and monitoring recovery from acute COPD exacerbations.
- Support patients with COPD requiring NIV, both new starts and during exacerbations.
Essential criteria
- Registered Nurse & current NMC Registration
- Specialist post graduate qualification or equivalent experience
- Proficient level of knowledge in clinical speciality Knowledge of service improvement
- Experience in the relevant specialty
- Experience of working in a multiprofessional environment
- Experience of Community working
- Able to demonstrate effective leadership and management skills
- Evidence of undertaking presentation to groups.
- Awareness of professional issues and developments
- Ability to use electronic systems - IT skills
Desirable criteria
- Teaching qualification (or equivalent experience
- Advanced clinical skills Non-medical Prescribing
- Experience of participating in practice change

Job summary
We are looking for dynamic, visionary and hard-working candidates to join our expanding Respiratory Department. We are advertising a full time post for excellent candidates to join the COPD Team. The team consists of Band 6 and 7 Respiratory CNS's and Respiratory Specialists Physiotherapists. The team will be responsible for identifying and assessing inpatients in hospital. Suitable in-patients with an exacerbation of COPD who are from the Swansea Bay area will be supported at home by this team. The team also works with patients ,GPs and Virtual Wards to prevent avoidable admissions .The successful candidate will also support patients requiring Non Invasive Ventilation ( NIV ) in the Community
