Role summary
The Senior Occupational Therapist will join the Home First Hub, an integrated discharge team, to provide reablement assessment to patients, predominantly with cognitive impairment, across a 5-day service. This role involves working with acute hospitals and visiting patients in short-term residential placements to understand their long-term needs and goals. The therapist will contribute to high standards of healthcare by assessing, planning, implementing, and evaluating care packages, focusing on patient recovery and independence.
Essential criteria
- Graduate Diploma or 1st Level degree in Occupational Therapy
- Current HCPC registration

- Clinical Educators qualification
- Previous experience of elderly rehabilitation
- Evidence of continuing professional development
- Experience of community working
- Evidence of a higher level of skills and knowledge of practice.
- Good communication and interpersonal skills
- Highly motivated and team player
- Able to demonstrate tact, diplomacy, empathy and sensitivity
- Good observational & reporting skills
- Ability to manage stressful situations and use own initiative
- Leadership skills
- Work flexibly to accommodate patient / service needs
- Have basic IT and standard keyboard skills
- Be able to kneel, bend & stoop, and work in cramped environments
- Be able to manoeuvre limbs and patients using handling aids

Job summary
Home First Hub is an integrated discharge team working with our Acute hospitals across Norfolk to support patients through their discharge home, community inpatient unit or to residential placement.
It is a multidisciplinary team of clinical and social professionals dedicated to patient recovery and wellbeing. EEC is seeking an experienced and enthusiastic Occupational Therapist to join our team in providing reablement assessment to a specific cohort of patients across a 5 day service Monday to Friday.
This role will focus on patients who are not able to immediately return home or enter a community in patient unit but require a period of recovery, reablement, and assessment to understand their long term needs and goals. It will involve linking with our team at Norfolk & Norwich University Hospital and visiting patients in their short term placements in residential homes across Norfolk. The specific cohort of patients will be those predominantly with cognitive impairment
