Role summary
This role offers an experienced Band 5 or Band 6 nurse the opportunity to specialise in heart failure within the Integrated Heart Failure Service (IHFS). The post holder will work with the Clinical Lead and Clinical Nurse Specialists to assess, plan, and implement specialist nursing care for community patients with heart failure. Key responsibilities include medication titration, admission prevention, and supportive discharge. The successful candidate will be committed to teamwork, possess good communication, and strong decision-making skills.

- Experience of formal/informal teaching
- Understanding of patient pathways and its complexities and how to support patient through this

- Flexible and adaptable to changing situations due to workload and colleague's needs
- Ability to undertake teaching of staff and patients
- Ability to work unsupervised and manage own workload; effective time-management

- Proven experience in supervision of trained staff and students

Job summary
This post offers the successful candidate the opportunity to develop the specialist knowledge and skills required to support heart failure patients within the Integrated Heart Failure Service (IHFS).
