Role summary
This role involves working as part of a multidisciplinary Haemoglobinopathy team to improve the health and wellbeing of individuals with haemoglobin disorders. The practitioner will develop and deliver health promotion initiatives, provide culturally appropriate education, and support self-management. The aim is to empower patients, reduce health inequalities, and improve access to healthcare services. This is a fixed-term contract for 22 months.
Key responsibilities
- Develop, deliver, and evaluate health promotion initiatives.
- Provide health education and self-management advice to prevent exacerbation of conditions.
- Address social and lifestyle factors impacting condition management and signpost to local resources.
- Build trust and rapport with patients while maintaining professional boundaries.
- Provide an In-reach service to secondary care.
- Organise patient-led education sessions and involvement with peer support programmes.
Essential criteria
- Degree or equivalent qualification in Health Promotion, Public Health, Health Education, Nursing, Allied Health Profession, or a related discipline.
- Evidence of continuing professional development
- Experience of delivering health promotion or patient education programmes.
- Experience working with individuals from diverse cultural and ethnic backgrounds.
- Experience working within healthcare, community health, or voluntary sector settings.
- Experience facilitating group education sessions and community engagement activities.
- Knowledge of safeguarding, confidentiality, and patient-centred care
- Excellent verbal and written communication skills
- Ability to engage effectively with diverse communities
- Ability to motivate and empower patients to improve self-management
- A valid full driving licence for the UK and access to a vehicle that can be used for work purposes.
- Good organisational and time management skills
- Competence in Microsoft Office and electronic patient record systems
- Ability to work independently and as part of a multidisciplinary team
Desirable criteria
- Experience working with people living with sickle cell disorder, thalassaemia, or other long-term conditions.
- Understanding of haemoglobinopathies, particularly sickle cell disorder and thalassaemia
