Role summary
The North West Integrated Neighbour Team is seeking a Band 6 Community Physiotherapist to join their team at Hartismere Hospital. This role involves delivering high-quality care in patients' homes, managing a diverse caseload, and formulating individual treatment plans. The successful candidate will utilize advanced assessment and clinical reasoning skills to ensure effective interventions and timely discharge. Excellent interpersonal and communication skills are essential for collaborating across professional boundaries.
Essential criteria
- Knowledge of additional specialist areas through in-house training & short courses
- In-depth knowledge of working within the NHS
- Able to demonstrate good IT and standard keyboard skills
- Must hold a full valid driving licence and have access to a vehicle or viable alternative
- Documented evidence of continuing professional development.
- Post registration experience in a relevant clinical setting.
- Advanced observational & reporting skills

- Evidence of previous community experience
- Able to demonstrate the management of complex casework
- Diploma or 1st Level degree in Physiotherapy (or equivalent)
- Current HCPC registration
- Clinical educators course within the first year

Main duties of the job
- To effectively triage and schedule all therapy referrals to the service in accordance with Trust's policies and procedures and advise junior colleagues on more complex issues.
- To manage a caseload of patients with a range of multiple and complex health needs.
- To utilise a high level of clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.
- To formulate an individual management/treatment plan for each patient using wide range of advance
