Role summary
The Senior Health Care Support Worker will be a vital part of the multidisciplinary Intestinal Failure and Rehabilitation team. Under the supervision of specialist nutrition nurses, the role involves delivering safe and effective patient care for individuals with Intestinal Failure. Key responsibilities include managing patient records, coordinating nurse activities, and facilitating patient pathways. The post holder will also ensure timely provision of supplies and manage the Nutrition Nurse Advice Line.
Essential criteria
- Functional skills minimum level 1 Maths and English assessed via BKSB (Basic and Key Skills Builder Aptitute Test or GCSE equivalent
- Completed Care Certificate
Desirable criteria
- Level 2 or 3 Health Care Qualification/apprenticeship, or completion of Band 3 Excellence in care.
- Competencies for existing Band 2 substantive HCSW staff who have worked in the trust at least 15 months
- Knowledge & experience
- Experience of working in acute hospital setting as a Band 2 HCSW role of equivalent
- Experience of speaking with patients and healthcare professions via the phone
- Knowledge of basic IT systems with experience of using computers
- Experience with NHS Digital Systems and willing to undertake training in local computer systems
- Skills, abilities and attributes
- Understanding of responsibility, accountability and confidentiality relevant to the Senior Health Care Worker role
- Have a basic understanding of the NHS and roles of nurses, doctors, dietitians and pharmacists within the Intestinal Failure Rehabilitation Teamabilitations
- Able to communicate clearly and concisely over the phone, face to face and in writing
- Evidence of continuing professional development
- Previous experience of working with patients with complex health needs
- Good attendance record

Job summary
The Senior Health Care Support Worker plays a vital role within the multidisciplinary Intestinal Failure and Rehabilitation team. Under the guidance and supervision of the specialist nutrition nursing team the post holder will be responsible for providing services in accordance with the Trusts values and behaviours in the delivery of safe and effective patient care for patients with Intestinal Failure.
Main duties of the job
Successfully manage the Intestinal Failure Database and Registry, maintaining accurate patient records and supporting service reporting requirements.

Coordinate Nutrition Nurse activities and clinic schedules to ensure efficient service delivery.
Facilitate patient pathways by creating and managing encounters for the Nurse-Led Rapid Access Clinic.
Ensure timely provision of ancillary supplies and equipment for patients receiving IV therapies.
Lead the administration and follow-up of the Nutrition Nurse Advice Line, ensuring all calls are documented, actioned, and resolved appropriately.
