Role summary
This role involves contributing to clinical rehabilitation and intermediate care for community patients under the direction of qualified practitioners. You will assist with administrative support and implement planned intermediate care/rehab in various settings, including patients' homes and community hospitals. The position requires active participation in learning opportunities and effective communication with service users and families. Working hours are between 08:00 and 20:00, covering 7 days a week on a shift pattern, including weekends and bank holidays.
Essential criteria
- Primary or Social NVQ level 2/3 or equivalent award in an appropriate field
- oDemonstrable competence in skills for role
- oExperience in a care based workplace
- oExperience in care of the older person
- oDemonstrate enthusiasm for providing high standards of patient care

- oWorking within the NHS
- SKILLS, ABILITIES & KNOWLEDGE
- oAbility to communicate effectively with patients/clients despite barriers to understanding that enables them to participate in their therapy
- oAbility to work autonomously and manage workload
- oBasic computer skills for example entering numbers in a spreadsheet or using email
- TRANSPORT
- oFull valid UK Driving Licence
- oCar owner
- EDUCATION, QUALIFICATIONS & TRAINING

Job summary
To contribute to the teams role in providing clinical rehabilitation and intermediate care to community patients under the direction of qualified practitioners.
To assist in the provision of administrative support to the department / team.
Implement planned Intermediate Care/Rehab for individuals in their own home, Community Hospital ward and other community settings maintaining a high standard of evidence based practice to meet identified needs.
Actively participate in learning opportunities to maintain knowledge and inform practice. Record own learning and personal development. Identify and articulate areas for self-improvement, including through self-reflection, evaluation of practice, and actively seeks feedback.
Communicates through a variety of methods, including the use of interpersonal, written and verbal and non-verbal skills. Exchanges information with service user and families using active listening, compassion, empathy, and coproduction techniques with the service user.
Hours of work are between 08.00 and 20.00 in shift pattern to cover 7 days a week. The post holder will be required to work weekends and bank holidays on a rota basis.
