An exciting opportunity has arisen for highly motivated, enthusiastic and suitably qualified and experienced Nurse Practitioner to join the Hospital at Home Team in NHS Forth Valley. This is a developing service and a unique opportunity to join our team and grow your career with us.

As part of the multidisciplinary team the nurse practitioner will be a specialist practitioner whose main role is to supplement the Medical staff capability to manage acute admissions to Hospital at home providing high quality, streamlined and evidence based care.

In conjunction with the multi-professional team the post holder will be responsible for the assessment of care needs and timely intervention
