Role summary

This is an exciting fixed-term opportunity for a Band 6 nurse to join the existing OPAT team. The role involves assessing individual care needs for patients requiring ongoing intravenous (IV) antibiotics at home post-discharge. Job summary

Main duties of the job

Complete competency assessment duties of patients from the emergency portals, out-patient clinics and the wards. With the support provided it will enable the patient to access the correct pathway to avoid admission and reduce length of stay.

Collate and format requested data.

Support admission portals and inpatient wards with knowledge and understanding of both intravenous and oral antibiotics as per advise from the MDT and advances in Antimicrobial Stewardship.

(AMS)Assess patients progress, review weekly/twice weekly blood monitoring and liaise with the MDT or referring consultant for additional support and advice.
