Role summary
This permanent part-time Community Staff Nurse role is within the District Nursing Service in Stockport, working 22.5 hours per week. The post holder will provide first-class care to patients in their homes and residential settings, supporting those with complex needs and their carers. The role requires a flexible, innovative practitioner with strong organisational and assessment skills, working as part of a multidisciplinary team.
Key responsibilities
- Work with Community Nurses and multidisciplinary teams to provide patient care.
- Provide care in patients' homes and residential homes, caring for patients with complex needs and supporting carers.
- Demonstrate good organisational and assessment skills.
- Be flexible and willing to cover the service between 08:00 - 18:00 over a 7-day period.
- Prioritise tasks and demonstrate good delegation skills.
- Maintain up-to-date knowledge on relevant nursing issues and service changes within Stockport.
Essential criteria
- Current 1st Level Registration with NMC
- Grade C/4 (or equivalent) in GCSE English
- Demonstrate knowledge of current local and national health agenda and developments
- Awareness of current clinical issues in out of hospital care
- Time management
- Ability to work under pressure
- Ability to work in a team
- Use own initiative
- Computer literate
- Demonstrate an awareness of patient dignity and respect.
- Flexible and adaptable
- Delivery of patient care and working in a clinical area.
- Occupational Health Clearance
- Due to the nature of the role, it is essential that applicants are able to travel independently across Stockport and have access to a suitable vehicle for business purposes. Where possible, adjustments can be considered in accordance with the Equality Act.

Desirable criteria
- Teaching and Assessing Certificate
- Evidence of Health-related study at level 6/7
- Diploma in Nursing Practice or degree
- Demonstrate an understanding of an implication of an issue which could impact on community service provision.
- Evidence of teaching and assessing students and junior members of staff
- Developing areas of expertise in community related practice. Tissue Viability/ Frailty/ end of life care
- Experience of audit involvement in project work.
- Emotional effort. The post holder will be exposed to frequent distressing and occasionally highly distressing and emotional circumstances
- May be exposed to highly unpleasant working condition/some exposure to hazards.

Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time".We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers.
