Role summary
This role is for a dynamic staff nurse to join the Responsive Services Team at First Community Health and Care. You will conduct assessments and treatments in patients' homes, including IV medications and wound care, working collaboratively with multi-disciplinary teams to prevent hospital admissions and support patient independence. Key responsibilities
- Work under the direction of the Responsive Services Charge Nurse/Sister as part of the Responsive Services team.
- Collaborate with Primary Care, Secondary Care, Community Nursing, and other Specialist Health Care Teams.
- Deliver excellent standards of patient care, assessing and treating acutely unwell patients to avoid hospital admission or support discharge.
- Provide practical care, education, and training for patients and carers admitted to responsive services.
- Complete assessments in patients' homes and undertake treatments including IV medications, wound assessment, and management.
- Work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home.
Essential criteria
- Evidence of time management skills and ability to prioritise

- Current NMC registration
- Registered First Level Nurse
- Educated to degree/diploma level Mentorship, ENB 998, practice assessor or equivalent

- Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.

consists of the Urgent Community Response Team, Intermediate Care Team, Virtual
Ward and Referral Hub.
