Role summary

This is an exciting opportunity for an experienced Diabetes Specialist Nurse and Independent Prescriber to join the Community Diabetes Team. The role combines advanced clinical practice with leadership of an innovative Continuous Glucose Monitoring (CGM) and insulin optimisation programme. The successful candidate will work collaboratively across primary, community and secondary care to drive service innovation and improve patient outcomes. This role involves providing expert clinical leadership, specialist diabetes management, and education for patients and healthcare professionals. It also includes undertaking comprehensive assessment and optimising diabetes therapies.

Essential criteria

  • Diploma/degree in nursing or equivalent experience
  • Post registration Diabetes Qualification.
  • V300 Nurse Prescriber

Desirable criteria

  • MSc in relevant topic
  • DAFNE qualification
  • DESMOND/X-Pert or similar qualification
  • Professional Registration
  • NMC Registration
  • Specific Skills
  • Up to date knowledge of current NHS Agenda.
  • Excellent knowledge of diabetes and its management.
  • Excellent patient management and assessment skills.
  • Report writing skills.
  • Teaching skills - ability to give presentations/group sessions
  • Experience of writing policies and procedures
  • Experience of clinical audit
  • Substantial post registration to include specialist diabetes experience.
  • Experience in developing and/or delivering teaching programmes.
  • Significant previous experience as a Diabetes Nurse
  • Experience of working in community, primary care settings.

  • Computer literate, confident in word and e-mail.

An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse and Independent Prescriber to join our forward-thinking Community Diabetes Team. This unique role combines advanced clinical practice with leadership of an innovative Continuous Glucose Monitoring (CGM) and insulin optimisation programme, supporting people living with diabetes to improve glycaemic control, increase independence and enhance their quality of life. Working collaboratively across primary, community and secondary care, the successful candidate will play a key role in driving service innovation, improving patient outcomes and shaping the future delivery of integrated diabetes care across Bromley.
