Role summary
This role is for an autonomous Registered Nurse responsible for assessing and treating patients with complex health and social care needs within a Hospital at Home setting. The post holder will provide holistic nursing assessment and high-quality, evidence-based care to maintain patients in their own homes, including physical examination and treatment planning. They will also work rotationally within a 24/7 telephone response service for Palliative and End of Life Care, acting as a keyworker for patients in virtual wards. The role focuses on admission avoidance and supporting timely discharge from acute hospitals.

Job summary
As an autonomous practitioner the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination and detailed history taking, diagnosis of deterioration inpre-existing conditions and treatment planning. Your experience and skills should enable you to recognise that a person is deteriorating and advanced care planning might be appropriate and/or that the person is coming into the last days of life and that dying phased care is appropriate.
Main duties of the job
The post holder will rotationally work within the Telephone response service for Palliative and End of life Care providing a 24/7 working closely with admission avoidance services to ensure patients safety is maintained.
