Role summary
This role involves coordinating care for patients with complex health and social care needs within integrated neighbourhood teams. You will manage a caseload, respond to referrals, and provide advice to patients and carers. The aim is to help patients live independently, reduce hospital admissions, and empower them to manage their health.
Key responsibilities
- Support housebound patients living with long-term conditions.
- Help reduce avoidable hospital attendances and admissions.
- Respond to referrals and coordinate access to services.
- Provide advice and support to patients, carers, and colleagues.
- Develop personalised plans that promote independence and prevent care crises.
- Empower patients to proactively manage their health and wellbeing.
Essential criteria
- You will have either: Level 4 qualification (e.g. certificate of higher education, higher national certificate, NVQ level 4) o Or you will be able to demonstrate the equivalent level of experience.
- You'll need to have recent experience of team working and versatility to take on a variety of tasks.
- A full, valid UK driving licence or a full driving licence issued by an EU country
- You'll need have sound communication skills in order to understand and communicate both verbally and in writing. Which at times may also require gaining cooperation from others through tact and persuasive skills particularly where there are barriers to understanding
- You will need to have a customer focussed approach which is essential to; Provide patients, clients and their families with the best possible experience
- You will need to be able to be able to use IT systems and databases
- You will have excellent attention to details, accuracy and organisational skills. You will know when you are within your own limits and when to escalate
Desirable criteria
- Having an awareness of developments within the NHS would be an advantage.
