Role summary
This role is for a Community Health Care Support Worker within the Urgent Community Response team in Hillingdon. The successful candidate will provide high-quality, patient-centred care and rehabilitation to patients in their own homes, supporting registered nurses and allied healthcare professionals. The position involves developing clinical skills like venepuncture and wound care, and requires a confident individual able to work autonomously under supervision. A full UK driving licence and access to a car are essential for travelling across the borough.
Key responsibilities
- Deliver good quality, patient-centred care to patients in the community.
- Support registered nurses and other allied healthcare professionals in delivering high quality, clinical care and rehabilitation with patients in their own homes.
- Practice confidently and safely within the community, utilizing skills such as venepuncture, wound care management, and catheterisations.
- Build good relationships with patients, their families and carers.
- Work in partnership with the multi-disciplinary Integrated Care Team.
- Work on a rota system, including weekends, for a service operating 7 days a week (08.00hrs - 22.00hrs).
Essential criteria
- NVQ 2/3 or equivalent
- GCSE English and Mathematics level C or above
- Previous experience of working with a multidisciplinary team
- Experience of providing care in a clinic setting
- Experience of working with older people
- Excellent time management skills, with the ability to work effectively under pressure, meet deadlines and prioritise workload
- Ability to work effectively as a team player under appropriate supervision, and as part of a multi-disciplinary team
- Ability to communicate with members of the public and health and care providers
- Able to communicate with people who are in pain or distress with calm and compassion
- Ability to move between sites working across health and social care as required by the needs of the development programme
- Be able to identify risks to patients and seek guidance from Team Leader/ Deputy Team Leader
- Ability to work on own initiative
- Ability to take part in reflective practice and clinical supervision activities
- Good inter-personal skills
- Ability to understand and implement the Equal Opportunities Policy at a level appropriate to the job.
Desirable criteria
- Foundation degree in nursing or equivalent
- Experience of working with people with dementia or other cognitive impairments
- Experience of supervision in practice as a supervisee
