Role summary

This role involves working as an integral member of the Enfield Adult Community Rehab Team, supporting clinicians and visiting patients in their homes. The post holder will carry out prescribed treatments, assess for and provide appropriate equipment, and work autonomously within team policies. Responsibilities include planning and delivering patient care, undertaking clinical work delegated by therapists, and managing a designated caseload.

Essential criteria

  • Relevant qualification e.g. C & G Health & Social Care NVQ 3 or equivalent
  • Training courses relevant to care group
  • English qualification at GSCE or equivalent level.

Desirable criteria

  • Trusted Assessor
  • Experience at working at band 3 level in a physical rehabilitation setting or experience within a social care setting
  • Knowledge of the ranges of equipment relevant to clinical area
  • Knowledge and experience of working with patients who have physical health disabilities / or impairments
  • Evidence of working with and caring for adults either in an acute, community NHS or social care setting.
  • Experience of using Rio
  • Ability to work autonomously & set own priorities
  • Insight into the needs of patients with long-term conditions and their carers
  • Ability to organise & respond effectively to complex information
  • Others
  • A full UK driving licence Car owner/driver for work purposes
  • Ability to fulfil routine duties, both physically and mentally, including all aspects of moving and handling patients.

Job summary

The post holders will be an integral member of the Enfield Adult Community Rehab Team. 1 role is 37.5 hrs, 2nd role is 18hrs 45mins.

The post holders will be developed to take short term responsibility for the effective planning and delivery of care to patients in co-operation with other team members and in communication with other clinical team leaders.

The post holder will be required to support clinicians staff with oversing education programmes and also to visit patients in their own homes to carry out prescribed treatment and assess for and provide appropriate O/T equipment or mobility aids directly or by prescription.

The post holder will work alone and will be required to use a degree of initiative while working within the parameters of the team's policies and procedures.

The post holder may be required to work at times in other services dependent on integration and service need.

The post holder will be an integral member of the multi professional teams, which is in partnership with the voluntary sector in delivering and their carers.
