Role summary
This is an opportunity for an enthusiastic and motivated pharmacy technician to join the Manchester and Trafford Local Care Organisation's community medicines optimisation service. The role involves providing patient-facing clinical pharmacy services, supporting urgent care, crisis response, and discharge to assess services. Key responsibilities
- Providing input as part of the medicines support to community urgent care service.
- Undertaking domiciliary visits to patients for medicines optimisation interventions.
- Working within a multidisciplinary team (MDT).
- Supporting the delivery of medicines optimisation to Discharge to Assess and Crisis response.
- Carrying out medication and compliance reviews with patients in area of clinical competence.
- Identifying and acting on medication errors/queries within area of competence.
Essential criteria
- Registration with GPhC
- Pharmacy Technician - BTEC in Pharmecutical Sciences or equivalent
- A significant amount of experience working as a pharmacy technician in hospital or community pharmacy
- Experience of implementing change and new developments

- Experience of working in a hospital as a ward based technician

Manchester Community Urgent Care services
This is a fantastic opportunity for an enthusiastic and motivated pharmacy technician to join the well supported and award-winning Manchester and Trafford Local Care Organisation, community medicines optimisation service (CMOS). The service provides patient facing clinical pharmacy services, working closely with other pharmacy teams and services across Manchester Foundation Trust.
The LCO Urgent Care Crisis Response and Discharge to Assess (D2A) services play an important role in supporting the NHS 10-year plan to deliver more care in patients' homes and community settings. The D2A service supports individuals recently discharged from hospital to remain safely at home. The Crisis Response service provides timely, responsive care to patients at home, helping them avoid unnecessary attendance at hospital emergency departments and other healthcare services. This model of care is delivered by an experienced multidisciplinary team (MDT), including advanced care practitioners (ACPs), nurses and pharmacists.
To apply for this post, you must be a General Pharmaceutical Council (GPhC) registered pharmacy technician with substantial experience of working within primary or secondary care. You should be confident to work independently and have excellent organisational and interpersonal skills. You must have a full UK driving lic
