Role summary

As a Community Staff Nurse, you will deliver high-quality, person-centred care to patients in their own homes and community settings. You will support patients to maintain independence and achieve the best possible outcomes, working alongside district nursing teams and other health and social care professionals. This role involves providing skilled clinical interventions and supporting patients, families, and carers throughout their care journey. A full UK driving license and live NMC UK pin are required.

Key responsibilities

  • Deliver skilled nursing care to patients within the community, ensuring safe, effective and person-centred care
  • Assess, plan, implement and evaluate care programmes in partnership with patients, families and carers
  • Manage and prioritise a delegated workload, responding to both planned and unplanned patient needs
  • Support patients to maximise their health, wellbeing and independence through clinical interventions, education and health promotion
  • Maintain accurate clinical records and ensure care is delivered in line with professional, legal and organisational standards
  • Work collaboratively with multidisciplinary teams, statutory agencies and voluntary organisations to coordinate care and support discharge planning
  • Provide support, supervision and mentorship to students, newly qualified staff and junior team members

Essential criteria

  • Current NMC registration as a First Level Registered Nurse or equivalent registration with relevant nursing experience
  • Evidence of continuing professional development and commitment to lifelong learning
  • Strong communication and interpersonal skills with the ability to build effective relationships with patients, carers and colleagues
  • Ability to work independently, use initiative and make sound clinical decisions within a community setting
  • Effective leadership, organisational and workload management skills
  • Good IT skills including experience of using email, electronic records and digital systems
  • Proven ability to work collaboratively as part of a multidisciplinary team while delivering high-quality patient care
