Role summary
This role involves supporting the delivery of quality healthcare interventions to patients in their own homes within the community. You will help meet their health and wellbeing needs and facilitate re-ablement support to maximize functional independence. The position is part of the Shropshire, Telford & Wrekin Virtual Wards initiative, aiming to provide rapid assessment, treatment, and virtual monitoring as an alternative to hospital admission. You will work with clinical teams to deliver effective patient care and build relationships with healthcare professionals and patients.
Essential criteria
- GCSE Grade A-C in English & Maths or equivalent.
- NVQ Level 3 in Direct Care or equivalent.
- Venepuncture Certificate of Competency or willing to work towards.
- Demonstrable experience in a healthcare setting.
- Ability to work with MS Office package or willingness to work towards.
- Knowledge and Skills
- Awareness of long-term conditions.
- Awareness of needs of palliative care patients.
- Experience in performing basic nursing tasks such as simple dressings.
- Experience in the administration of insulin and monitoring of blood sugars, or willing to undertake additional training.
- Able to work independently with guidance.
- Remains aware of own limitations.

The Virtual Ward is a system wide initiative, working in partnership to provide our patients with access to rapid assessment, treatment, virtual monitoring and care planning to stabilise and support an acute episode of ill health or a deteriorating chronic condition that would alternatively require admission to hospital.
The service offers an appropriate alternative to admission pathway, which aims to deliver a recommended and agreed medical, nursing & therapeutic treatment plan. The focus is supporting the safe management of people in their own home. The role and service offer integration and partnership working across the system to ensure a seamless service.
