Role summary
This role involves working as a Home Treatment Practitioner within a Multi-Disciplinary Team, providing care plans and support to patients. You will promote compliance with treatments, develop partnerships with stakeholders, and advise patients and carers on available resources. The position requires an understanding of mental health legislation and the ability to act as a Lead Professional following CPA principles.
Key responsibilities
- Work as a member of a Multi-Disciplinary Team, reporting to colleagues verbally and in writing.
- Provide each patient with an agreed care plan as a Lead Professional, in accordance with Community Mental Health Nurse and Care Programme Approach standards.
- Actively contribute to setting standards of care and implement quality initiatives.
- Promote compliance with medication and other prescribed treatments, noting and reporting non-compliance and side effects.
- Develop and maintain healthy partnerships with statutory, third sector, and voluntary agencies.
- Advise patients, carers, and significant others of available resources, facilitating their use and referring assessed needs.
Essential criteria
- Registered Mental Health Nurse and current professional registration with the NMC or registered allied health professional.
- Evidence of Continuing Professional Development
- ENB 998 or other NMC approved mentor qualification
- Key skills in literacy, numeracy and IT Level 2 or equivalent
- Risk Assessment and Management Training
- Suicide Prevention Training
- Supervision Training
- Demonstrable post registration experience as a Registered Mental Health Nurse
- Professional experience of working in mental health
- Experience of MDT working
- Skills in coordinating and planning inter-agency approaches in mental health care
- Experience of assessing and managing risk
- Ability to use initiative and work without immediate supervision
- Ability to demonstrate an understanding, empathic approach
- Skills in co-ordinating and planning interagency approaches in mental health care
- An understanding of current mental health issues
- Knowledge and understanding of the legislative framework for mental health services, particularly the Mental Health Act and Mental Capacity Act
- Knowledge and understanding of the roles of other professionals working within mental health
- Excellent report-writing skills
- IT skills
- Ability to work as part of a multi-professional team
- Understanding of the CPA process
- Excellent assessment skills, including risk assessment
- Understanding of the importance of user/carer involvement in care planning
- Experience of Safeguarding
Desirable criteria
- Experience of working in the community.
- Previous experience within an assessment / liaison workplace.

Job summary
37.5 hours per week - 08:00 to 21:30 hours 365 days a year
To work as a member of a Multi-Disciplinary Team reporting back to colleagues, both verbally and in writing and seeking advice, guidance and support where necessary.
To provide each patient for whom they are a Lead Professional with an agreed care plan, containing review dates, in accordance with Community Mental Health Nurse and Care Programme Approach standards.
To actively seek to contribute to setting standards of care, to generate and implement quality initiatives.
To promote compliance with medication and other treatments prescribed, noting and reporting non-compliance and side effects as appropriate.
