Role summary
This is an exciting opportunity for a Community Psychiatric Nurse to join the dynamic Home Treatment Crisis team at Slough OPMH. The role involves providing high-quality treatments and working with the clinical team to assess and implement intensive care for people with mental health needs/dementia in the community. You will manage a caseload, undertake patient assessments, and formulate care plans. The position requires specialist knowledge of older people's mental health problems and crises, and the ability to travel within the community.
Key responsibilities
- Participate as a member of a multi-disciplinary team managing a caseload of patients.
- Undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
- Be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients.
- Direct and supervise Support Workers and/or students.
- Provide advice to other members of the service and agencies on the needs of service users.
- Provide a fast and flexible response to care needs, responding to referrals in a timely manner and providing intensive treatment.
Essential criteria
- Registered Mental Health Nurse
- NMC Registration
- Specialist knowledge acquired through post graduate short courses, training and equivalent experience, in a relevant field.
- Evidence of continuing professional development
- Demonstrable experience of working autonomously to deliver high quality and evidence-based nursing care.
- Experience in providing supervision to junior colleagues and students in the completion of routine nursing and clinical skills.
- Uphold the Trust's values of being caring, committed and working as team through demonstrable examples and experience.
- Demonstrable understanding of an acute mental health environment and relevant legislation such as the Mental Health Act, Mental Capacity Act and DOLs
- Proficient use of digital tools and applications including Word, Outlook and patient management systems.
- Ability to travel between multiple Trust sites and non-Trust locations, including patient homes, some of which may not be easily accessible via public transport.
