Key responsibilities
- Work with patients and their families who have complex needs and conditions to support timely, safe and effective discharge into the community.
- Provide professional guidance and support to patients and carers throughout the discharge planning process.
- Develop and coordinate holistic discharge plans in partnership with patients, families and multi-agency colleagues.
- Ensure discharge interventions are appropriate, coordinated and not duplicated across services.
- Facilitate smooth, safe and effective transfers of care from hospital to home and between community and acute settings.
- Contribute to improved quality of care and patient satisfaction through effective discharge coordination.
Essential criteria
- NMC Registration
- Evidence of proactive and continuous personal and professional development
- Demonstrable post registration, which must include a strong experience in an acute trust
