Role summary
This role is for a part-time Community Mental Health Practitioner within the CMHT at Horton House, Oldham. The successful candidate will act as a Care Coordinator, providing comprehensive mental health services to adults with moderate to severe mental health problems. The practitioner will manage a defined caseload, work collaboratively within a multidisciplinary team, and focus on recovery-oriented care for service users.
Key responsibilities
- Provide comprehensive mental health services to adults with moderate to severe mental health problems.
- Work collaboratively across disciplines to promote a 'needs based' approach for service users.
- Develop clear relapse recognition/prevention plans and share with MDT, service users, and carers.
- Engage meaningfully with individuals often referred to as 'difficult to engage' and their families.
- Manage the care of a defined caseload of patients within the CMHT.
- Work effectively with adult service users with mental health needs, predominantly covering the care coordinator function.
Essential criteria
- Professional qualification Social Work / Mental Health Nursing.
- Registration with HCPC / NMC
- Substantial experience in community mental health work.
- Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate.
- Working within Mental Health Law policies and procedures.
- Experience of supervision
- Experience in working with people with moderate to severe mental health needs
- Experience of MDT working
- Extensive knowledge / experience in social inclusion and recovery model of care.
- Knowledge of mental health legislation
- Ability to and be responsible for management of a caseload
- Ability to work effectively as part of a MDT.
- Ability to supervise junior staff / students
- Effective time management
- Excellent written and verbal communication skills
- Recording relevant documentation to a professional standard.
- Ability to communicate informally and develop supportive relationships with clients, carers and other professionals
- Utilise the supervision process.
- Ability to value diversity and respond constructively to discriminatory behaviour.
- Ability to provide professional expertise in the care of mental health service users.
- Ability to provide appropriate education and training to peers and assistant as appropriate.

We currently have a vacancy for a part time Community Mental Health Practitioner within our CMHT.
The post is based at Horton House, Hamilton Street Oldham
You will need to be a registered and experienced Social Worker to apply for this post.
The Care Coordinator will provide a comprehensive mental health service to adults of working age who experience moderate to severe mental health problems within the community. The practitioner will also work collaboratively across a range of disciplines to continuously promote a 'needs based' approach to all service users.
The practitioner will also promote an integrated approach to service delivery and develop a clear relapse recognition / prevention plans and share with MDT, service users and carers' where appropriate.
The practitioner will develop meaningful engagement with individuals who are often referred to as 'difficult to engage' and their families.
Main duties of the job
The successful applicant will work effectively with adult service users with mental health needs across the CMHT, predominantly in the MDT covering the care coordinator function and will manage the care of a defined caseload of patients.
