Role summary
This is an exciting opportunity for a Band 5 Community Nurse to join the multidisciplinary Care Community Team at Chester East. The role involves delivering high-quality nursing services and holistic assessments to patients, primarily in their own homes. You will work with various professionals to coordinate care, aiming to prevent unnecessary hospital admissions. This position is ideal for nurses new to community health looking to gain valuable experience.
Key responsibilities
- Undertake duties involving the delivery of high-quality nursing services.
- Conduct holistic assessments of patients, ensuring a person-centred approach.
- Work alongside various professionals to provide quality and seamless care to patients predominantly in their own home.
- Be proactive in coordinating patients' care needs as appropriate.
- Aim to avoid unnecessary admission to hospital or long-term care settings.
Essential criteria
- Registered General Nurse part 1 of the NMC register
- Evidence of recent training
- Multi Agency Working
- Awareness and understanding of the Public Health Agenda
- Evidence of research based/reflective practice
- Knowledge of local and national priorities.
- Ability to manage time effectively
- Good interpersonal skills
- Demonstrate highest standards and skill in nursing.
- Demonstrate working knowledge of community nursing and multi-disciplinary working
- Demonstrate knowledge and understanding of complex wound management in the community
- Demonstrate knowledge and understanding of End of Life care in the community
- Demonstrate knowledge and understanding of safe medication administration practises

You will need access to transport or have the ability to travel to a variety of venues as part of the working day. Mileage expenses are reimbursed in addition to your hourly rate.
