Role summary
This role is for a Children's Nurse to join the Children's Hospital @ Home team, providing specialist nursing care to children and young people with short-term health needs. The service aims to prevent hospital admissions and facilitate early discharge by offering acute assessment and interventions in the home environment. The nurse will work collaboratively with hospital medical teams and GPs, providing both in-person and remote clinical assessments. Training and development opportunities are available within a supportive team.
Key responsibilities
- Provide specialist nursing care, support, and co-ordination of care to children and young people with short term health care needs.
- Carry out specific health care assessments and/or interventions under supervision.
- Conduct initial assessment of patients with short term health needs by telephone and provide remote clinical assessment via telephone or video call.
- Perform autonomous clinical assessment of acutely unwell children in the home environment.
- Participate in the administration of medicines, including intravenous therapy.
- Work collaboratively with professionals to enable children/young people to be cared for at home whenever possible.
Essential criteria
- Registered Nurse (Child)
- Previous experience in Acute setting
- Evidence of continued professional development
- Experience of effective working collaboratively with professionals from other disciplines
- Evidence based practice
- To effectively communicate ideas and influence others (oral and written)
- To work independently and as part of a multi disciplinary team
Desirable criteria
- History Taking and Physical Assessment Course
- Previous Community Children's Nursing Experience

In collaboration with University Hospital Southampton NHS trust (UHS), the service works closely with the medical team at the hospital and GPs in the community to facilitate acute assessment with the aim to prevent & minimise admissions to hospital and facilitate early discharge where possible.
