Role summary
This Band 5 Community Nurse role is within the Ely Community Nursing Team, covering Ely and surrounding villages. You will work as part of a Neighbourhood team, collaborating with various healthcare professionals to provide excellent nursing care at home and reduce hospital admissions. The role involves keeping patients at the centre of care, providing high standards of nursing intervention, and working within a 24/7 multidisciplinary team. Independent travel with a full UK driving licence and access to a vehicle is essential.

We are looking for a Band 5 Community Nurse to work within our Ely Community Nursing Team covering Ely and surrounding villages
In these roles, you would be working at part of the Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses and Team coordinators. Linking closely with colleagues including general practitioners and other members of the multidisciplinary team to support excellent nursing care at home, reducing the necessity for acute hospital admission.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
